Prairie St. John's - Admission Process
Prairie St. John’s Needs Assessment team will answer your questions, arrange assessments and coordinate the admission process. Call (701) 476-7216 or (877) 333-9565 (Toll Free) 24 hours a day, 7 days a week to arrange for an assessment or admission for your referral.
When you call, please be sure to have the following patient information on hand to ensure a smooth admission process:
- Name
- Age
- Birth Date
- Responsible person or guardian (as appropriate)
- Presenting behavioral issues and problems (reasons for your referral)
- Safety Risk (suicidal/homicidal thoughts, risk of harming self/others)
- Active medical problems
- Past pertinent medical and psychiatric history
- Current medications, including those prescribed by a psychiatrist
- School records (if applicable)
- Copies of any psychological testing completed or mental health/chemical dependency evals
- History of any encounters with law enforcement agencies
In addition, please have the following insurance information available to allow our offices to research insurance coverage and minimize out-of-pocket expenses:
- Insurance Company Name
- Policy Number
- Group Number
- Effective Date of Coverage
- Policy Holder
- Group or Company Name of Policy Holder
